Nervous stomach: knots, butterflies and a pit in your stomach

Why anxiety goes straight to your stomach, what a nervous stomach feels like, how to calm it, and the symptoms that need a doctor.

Before a big event, a hard conversation or out of nowhere: a knot in your stomach, butterflies, a sinking pit, or a sudden rush to the bathroom. For many people, anxiety lives in the gut as much as in the mind.

A nervous stomach is uncomfortable, but it makes sense once you know how your gut and brain work together. Here’s why it happens and how to calm it.

Why anxiety goes to your stomach

Your gut has its own network of nerves, sometimes called the “second brain”, and it’s in constant contact with your brain, especially through the vagus nerve. That’s why emotions so often show up in your stomach.

When you feel anxious, your body’s fight-or-flight response kicks in:

  • Digestion is put on hold, as blood moves away from your gut to your muscles. That can cause nausea, a heavy feeling or loss of appetite.
  • Or things speed up, which can lead to cramps and a sudden need for the toilet.
  • Your stomach muscles tense, creating that knotted, clenched feeling.
  • Your gut becomes more sensitive, so normal digestion feels more noticeable, and more uncomfortable.

What a nervous stomach feels like

  • butterflies or fluttering
  • a knot, tightness or a “pit” in your stomach
  • feeling sick, or a sinking feeling
  • cramps or stomach ache
  • loss of appetite
  • needing the toilet urgently, or diarrhoea
  • bloating, gas or indigestion

Many people find these feelings make them more anxious, especially if they worry about being sick or not reaching a bathroom in time.

How to calm a nervous stomach

  • Breathe into your belly. Put a hand on your stomach and breathe in slowly through your nose so your hand rises, then breathe out for longer. This relaxes your diaphragm and stomach muscles.
  • Add warmth. A hot water bottle or warm compress on your stomach can ease cramps and tension.
  • Sip something warm, like water or caffeine-free herbal tea. Some people find ginger or peppermint tea soothing.
  • Move gently. A slow walk helps get digestion moving and uses up some of the adrenaline.
  • Eat little and often, choosing plain, gentle foods when your stomach is unsettled, rather than skipping meals entirely.
  • Go easy on caffeine, alcohol and fizzy drinks, which can all irritate a sensitive stomach.
  • Name it: “This is my nervous stomach. It’s uncomfortable, but it isn’t dangerous, and it will settle.”

When it keeps happening

If stomach symptoms come and go for weeks or months, with cramps, bloating, diarrhoea or constipation, it may be irritable bowel syndrome (IBS). The NHS lists stress and anxiety among the things that can trigger IBS flare-ups. A doctor can help, and calming anxiety often helps IBS too.

How Breatheasy can help

Breatheasy can help settle the anxiety behind a nervous stomach. Its guided breathing exercises pulse your phone as you breathe in, so you can close your eyes and follow along, and slow belly breathing is one of the quickest ways to relax your stomach. If anxiety turns into panic, tap “I’m feeling anxious” and Luma walks you through it, step by step. Ten-second check-ins and a journal help you spot what sets your stomach off. Breatheasy is coming soon to iPhone and Android.

When to see a doctor

See a doctor if stomach symptoms keep coming back or last for more than a few weeks. Get help urgently if you have:

  • weight loss you can’t explain
  • bleeding from your bottom, or blood in your poo
  • a lump or swelling in your stomach
  • vomiting blood, or severe pain

These need checking whatever you think the cause is. If anxiety is behind your symptoms, our guide to anxiety and panic attacks explains what helps.
